Purpose: This form provides families with a consistent process to request that a student be excused from specific curriculum, instructional material, lesson, activity, or other school-based instruction when an opt-out is permitted by law, Board policy, or district procedure. Submission of this form does not automatically approve a request. The district will review the request and communicate next steps.
